Greater Manchester key worker service

The Greater Manchester key worker service provides dedicated key workers for children and young people with a learning disability and/or autism diagnosis, who are at high risk of going into or are already in hospital or a placement away from home.

Our key workers help children and young people to access the right help and work closely with their families.

What will a key worker do?

Take time to get to know you, understand your needs and wishes and support you to make informed choices.

Make sure that you and your family have your voices heard and understood by other services.

Support you to make informed choices, and help you to shape your future goals.

Who is the Greater Manchester key worker service for?

A key worker is for a young person who

  • Is aged 0-25 years
  • Has a diagnosis of autism and/or learning disability
  • Is on the dynamic support register (DSR)
  • Is at risk of going in/is already in hospital or placement away from home.

What is the dynamic support register (DSR)

The DSR is for people who need a team approach to their support. It holds important information about children and young people. It is used to help services work closely together and making sure the right support is in place. You will have to give consent to be on the register.
