Education, Health and Care (EHC) Assessment and Review Team

About

The EHC Assessment and Review Team is responsible for carrying out EHC needs assessments for children and young people with high needs aged 0 to 25 years.

We liaise closely with parents, education settings and a range of services to ensure the provision made for those with Education, Health and Care Plans (EHCP) is both co-ordinated and matched to their need.

Meet the team

A local authority education and healthcare team is a group of people who work together to help kids who need extra help with their learning or health. They make plans to make sure these kids get the right support at school and with their health needs. This team includes teachers, doctors, and other experts who care about helping these kids do well and feel good.

Holly Davies - Assistant EHC Assessment and Review Officer

To oversee the Education, Health and Care needs assessment and annual review processes, participating in casework meetings and liaise with other agencies. 

Leanne Beaumont - Senior EHC Assessment and Review Officer

To assist the team manager in ensuring that the statutory requirements of the team are met.  Including supervision of the team, quality assuring EHCPs and attending multi-agency meetings.

Emma O'Donnell - EHC Assessment and Review Officer

To oversee the Education, Health and Care needs assessment and annual review processes, participating in casework meetings and liaise with other agencies. 

Clare Fleming - EHC Co-Ordinator

To respond to general enquiries, including travel assistance.  To be responsible for the processing of draft and final EHC plans and for the copying and circulation of all confidential reports to parents and agencies.  To maintain case files and to provide administrative support to multi-agency panels. 

Hannah Platt - Assessment & Reviewing Officer

To oversee the Education, Health and Care needs assessment and annual review processes, participating in casework meetings and liaise with other agencies. 

Donna Bennett - EHC Assessment and Review
