Learning Disabilities Transition Team
The transition team sits within the Learning Disability Team and aims to provide transition support for young people with learning disabilities (aged 17-19) who meet the learning disability team criteria for access to services.
We undertake assessments of young people’s social care and health needs in order to recommend and provide the appropriate service to provide support or equipment to help with daily living.
The team consists of a Social Worker, a Community Learning Disability Nurse and a Support Planner who work very closely with other agencies to assist the transition of young people across children to adult services.
